I know of no single tool, only a collection of tools. Just as a personal assistant performs different work depending on your needs, the software of choice is also different depending on your needs. This is a great topic to more fully expand, and LifeHacker.com spends a great deal of time looking at it from different angles. In the meantime, here are a few quick suggestions:
